Welcome to the Madison Health Office Page
from Mrs. Devitt and Mrs. Baker

Check the School District 203 Health page for
the general policies and downloadable forms plus lots of helpful information!

We are attempting to establish good attendance patterns in our students since it has been shown that there is a positive relationship between regular attendance, punctuality and school success.  Some children have been arriving at school after classes have begun in the morning.  Please assist us by having your child on the school grounds at 7:45 a.m. so that he/she is seated in the classroom and ready to begin instructional time at 8 a.m.

  • VISION EXAMINATIONS As of January 1, 2008, the State of Illinois has required that all students entering kindergarten or entering school for the first time from out of state shall have an eye examination before October 15th of the school year.
  • DENTAL EXAMINATIONS As of July 1, 2005, a new State of Illinois law went into effect requiring that all students in Kindergarten, second and sixth grades have an oral health examination performed by a licensed dentist. Following your child's exam, please have your dentist fill out the form required by the State  and return it to the Health Office of his/her school no later than May 15, of each school year. Exam forms are also available there.   In addition, if you need a referral to find a dentist or financial assistance to obtain your child's exam, please contact your school's Health Office for assistance from the nurse.  Click to print or view dental forms
  • EMERGENCY CARDS  MUST  be updated each year.  They provide the contact numbers in time of emergency.  Many families are on the move.  Please be sure to update parent work numbers and emergency contact phone numbers.  You may do so by sending a note to the Health Office.  This is also a reminder to parents who have occasion to go out of town and leave their children in the care of someone else.  Be sure to put in writing your authorization for emergency care during your absence, and to please notify the Health Office of the person who will be in charge.
  • REGARDING MEDICATIONS in school.  This  is a reminder that the school CAN NOT give medicine, prescription or over the counter, unless we have a signed permission form from your child's  doctor.  Medicine should be brought to school in the original prescription bottle (pharmacies will give an extra one when asked), or in the container in which it was purchased.  It should be clearly marked with the student's name and dosage to be given.  All medicine MUST be given through the Health Office, as it is not safe for your child or for other children to have medicine sent in lunch boxes or pockets.
  • IMMUNIZATIONS see District 203 Health Office Page
  • OUTDOOR RECESS - is held unless the temperature or the wind chill is 0°F or below.
  • SPORTS PHYSICALS - All students who try out for an interscholastic sport including  cheerleading, MUST HAVE a CURRENT PHYSICAL on file in the Health Office.  Physicals are good for one year.
  • STUDENT ABSENCES - Please remember that student absences MUST BE reported on the Attendance Line(630-848-5466) before 8 a.m. or 420-6400 after 8 a.m. each day that a student will not attend school.  It is especially important that anticipated absences, such as  those associated with holiday travel, be called in to the Office( 420-6400) with the date(s) the student will be gone.  Otherwise, the Office will be trying to contact you at home, work, or through emergency numbers to make sure your child is safe.  Please be sure to keep all numbers current by sending a note to the Office or by phone
  • COMMUNICABLE DISEASES  - Because many communicable diseases occur throughout the school year and because most do not pose a serious health threat to the general population, we do not inform parents every time a new case of illness occurs.  If  you have a special need to know about cases of communicable diseases such as chickenpox, measles, fifth's disease, etc. occurring in your child's classroom (a family member has sickle cell anemia ,is immuno-suppressed, is pregnant or has a similar health concern), please inform the Health Office if such an illness occurs in your child's class.  Please advise the Health Office should your child come down with a contagious illness, especially fifth's disease so that any pregnant teachers or volunteers can be notified.
  • INFLUENZA NOTICE - see Distict 203 Health Office Page or CENTER FOR DISEASE CONTROL page
