Transcripts & Records

Academic transcripts and/or immunization records may be requested by signed letter or telephone directed to:

Processing costs:

• Transcripts - $5.00
• Immunization records - $3.00

Information required to process requests:

• Full name of student at the time of attendance
• Date of Birth
• Year of graduation
• Current telephone number

Voice-mail messages must include the spelling of the former student's name and current telephone number.

Telephone requests for official transcripts to be sent to a school, employer, etc., must include the complete name and address of the inteneded recipient.

Requests for official transcripts to be sent to a home address must be in writing and signed by the former student.
